We are seeking an experienced Nurse Practitioner to join our team at the Spokane Area Post-Acute Care Facility. As a primary care provider, you will be responsible for delivering high-quality patient care in a post-acute setting. If you are passionate about patient care and looking for a challenging and rewarding role, we encourage you to apply.
About the Role:
- Perform primary care in a post-acute setting for short-term and long-term patients
- Manage chronic disease, diagnose illnesses and medical conditions, and implement a plan of care
- Order and interpret lab results
- Prescribe and monitor medications
- Consult with physicians and coordinate patient care with other healthcare practitioners
- Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
- Build relationships with patients, patients' family members, and facility staff to facilitate effective treatment delivery
- Promote healthy lifestyle changes
Requirements:
- 1+ years of experience as a Nurse Practitioner in a primary care setting required
- Post-acute care experience a plus, but not required
- Active and unrestricted Washington NP license required or application in progress
- Active and unrestricted Washington DEA required or application in progress
