Senior Living Care Clinician Opportunity

We are seeking a skilled Senior Living Care Clinician to join our team at UnitedHealth Group. As a Senior Living Care Clinician, you will be responsible for providing high-quality care to our patients in assisted living and other community-based settings.

Key Responsibilities:

  • Conduct comprehensive assessments of patients to identify their medical, social, and functional needs.
  • Develop and implement individualized care plans to address the unique needs of each patient.
  • Manage patients through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
  • Collaborate with interdisciplinary care teams to ensure seamless care coordination and communication.
  • Provide education and counseling to patients and their caregivers on disease management, medication adherence, and healthy lifestyle choices.

Requirements:

  • Active and unrestricted licensure as a Nurse Practitioner or Physician Assistant in the state of practice.
  • Graduate of an accredited Nurse Practitioner or Physician Assistant degree program.
  • Board certification by the National Commission on Certification of Physician Assistants (NCCPA) or equivalent.
  • Ability to obtain DEA licensure and prescriptive authority, if required by state regulations.
  • Ability to complete physical requirements of the job, including lifting, standing, and kneeling.

Preferred Qualifications:

  • 1+ years of clinical experience in senior living, geriatric medicine, or home care.
  • Experience in meeting the healthcare needs of patients with complex behavioral, social, and functional needs.
  • Understanding of geriatrics and chronic illness management.
