Overview
To deliver comprehensive nursing services to patients within a healthcare facility. Patient care encompasses various age groups: pediatric (infant to 12 years), adolescent (13 to 17 years), adult (18 to 64 years), and geriatric (over 64 years).

Position Details

LOCATION: Not specified

HOURS: Full-time, 1.0 FTE (80 hours/2 weeks); four days per week from 7:15 am to 4:30 pm; one day per week from 7:15 am to 12:15 pm; one evening per week; one weekend per month.

KEY RESPONSIBILITIES: Qualified candidates must possess the capability (with or without reasonable accommodation) to perform the following tasks:
  • Act as a subject matter expert and resource for all clinical personnel, providing guidance and support as needed.
  • Offer clinical advice via phone or electronic communication based on clinical assessments and triage protocols, adhering to regulatory standards and practices. Consult with providers as necessary.
  • Deliver patient-specific education covering preventive care, chronic disease management, acute diagnoses, medications, and other relevant areas impacting patient outcomes.
  • Welcome patients and escort them to examination rooms, preparing them for their consultations.
  • Following established protocols, verify patient information by interviewing them to gather essential medical details. Accurately document medical and social history and the purpose of the visit in the patient record.
  • Prepare patients for examinations by measuring vital signs, including pulse rate, temperature, blood pressure, weight, and height, and accurately record this information in the patient chart.
  • Review and update the current medication list, making necessary adjustments. Address medication concerns with patients and provide education on potential side effects.
  • Administer injections and medications as prescribed by the provider, following departmental protocols and regulatory requirements.
  • Assist providers with various procedures, minor surgeries, and examinations as directed.
  • Implement standing orders established by the medical group.
  • Conduct various tests at the request of the provider, including but not limited to hearing tests, EKGs, breathing treatments, and vision assessments.
  • Collect blood or other laboratory specimens and prepare them for further analysis.
  • Authorize prescription refills as directed by the provider.
  • Assist with tracking tests and monitoring overdue results.
  • Remove sutures and change dressings on wounds as necessary.
  • Ensure examination rooms are adequately stocked and cleaned after each patient visit. Maintain and sanitize medical instruments and equipment following each procedure.
  • Support providers and patients in reviewing and updating care plans, assessing patient progress, and following up as necessary.
  • Engage in proactive outreach for patients requiring preventive and chronic disease services.
  • Coordinate care activities among specialists and manage transitions of care to obtain all necessary patient information.
  • Assist in coordinating referrals, education, and community resources.
  • Actively participate in performance improvement initiatives aimed at enhancing overall patient experience and quality of care.
  • Participate in daily huddles with the provider.
  • Serve as a patient experience advocate, maintaining a positive demeanor when interacting with patients, visitors, and colleagues. Communicate professionally with all stakeholders while prioritizing customer service.
  • Handle all patient information confidentially, adhering to privacy and confidentiality policies and procedures.
ADDITIONAL RESPONSIBILITIES: The following tasks are considered secondary to the primary responsibilities listed above:
  • Provide input into performance evaluations for clinical staff as requested by management.
  • Undertake other duties and special projects as assigned by management.
  • Participate in on-call responsibilities as required.
  • Perform other duties as assigned.
MINIMUM REQUIRED QUALIFICATIONS:
  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) issued by the appropriate state board.
  • Two (2) to three (3) years of nursing experience in a primary or specialty care setting.
  • AHA CPR/BLS certification required within 30 days of hire.
PREFERRED QUALIFICATIONS:
  • Previous lead or supervisory experience in a clinical environment.
