We are seeking a compassionate and skilled Home Health Aide to join our team at Franciscan Health. As a Home Health Aide, you will play a vital role in providing high-quality care and support to patients in their own homes.

Key Responsibilities
  • Assist patients with daily living activities, such as bathing, dressing, and grooming, as directed by the Plan of Care.
  • Record and document patient care, including intake, output, and vital signs, as directed.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Assist patients with ambulation and transfer using proper body mechanics.
  • Schedule visits with patients or caregivers as needed.
Requirements
  • High School Diploma or equivalent required.
  • 1 year of experience in home healthcare or long-term care setting preferred.
  • Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification required.
  • CPR certification required upon hire.
  • Auto insurance and driver's license required upon hire.
