Key Responsibilities:
The Social Services Coordinator will play a crucial role in our Behavioral Health department. This position involves working closely with families to address developmental concerns for infants and toddlers. You will facilitate evaluations and collaborate with a team of specialists, including speech, physical, and occupational therapists, as well as social workers and developmental therapists.
Your responsibilities will include:
- Conducting intake assessments and coordinating eligibility processes.
- Assisting families in identifying appropriate service providers.
- Facilitating the creation of Individual Family Service Plans (IFSP).
- Maintaining accurate early intervention records.
- Updating and managing information within our systems.
- Ensuring ongoing communication with families and conducting regular IFSP reviews.
- Implementing transition plans for children moving to preschool or special education programs.
- Completing family fee schedules and conducting developmental screenings.
- Delivering public awareness presentations.
Qualifications:
- A Bachelor's degree in Social Work, Psychology, or a related field is required.
- A minimum of 1 to 3 years of experience in Early Intervention or working with families of children with special needs is essential.
- Candidates must obtain credentialing as a Service Coordinator.
- Maintaining automobile insurance is a requirement.
