Job Summary

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct comprehensive health assessments in the comfort of patients' homes, using our proprietary assessment tool to identify potential health risks and develop personalized care plans.

Key Responsibilities
  • Conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up.
  • Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits.
Requirements
  • Master's degree required or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • Minimum of 1 year experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with employee health/medical record software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.
  • Ability to travel, valid state driver's license, and adequate automobile insurance coverage.
