Home Health LPN Job Description

We are seeking a skilled and compassionate LPN to join our Home Health team at HCA Virginia Healthcare at Home. As a Home Health LPN, you will have the opportunity to provide high-quality patient care in the comfort of their own homes.

Job Summary

The Home Health LPN will administer quality, skilled nursing care to patients in their homes, working in accordance with physician orders and the Plan of Care under the direction and supervision of a Registered Nurse and/or Director, Professional Services.

Responsibilities
  • Administer medications and treatments as ordered by the physician
  • Perform vital signs and other assessments as needed
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with other healthcare team members to ensure seamless care coordination
  • Document patient progress and communicate with the healthcare team as needed
Requirements

To be successful in this role, you will need:

  • A current LPN license in the state of Virginia
  • At least 1 year of experience in home health care or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
