Job Title: Habilitation Specialist

At Adapt Community Network, we are seeking a highly skilled and dedicated Habilitation Specialist to join our team. As a Habilitation Specialist, you will play a critical role in designing and implementing individualized programs for individuals with developmental disabilities, promoting their independence and community participation.

Key Responsibilities:
  • Develop and implement individualized programs for individuals with developmental disabilities
  • Provide supervision and guidance to Habilitation Assistants
  • Collaborate with families, residences, and other stakeholders to ensure seamless service delivery
  • Conduct regular assessments and evaluations to inform program development and implementation
  • Ensure compliance with regulatory requirements and agency policies
Requirements:
  • Bachelor's degree in a human services field
  • One year of post-graduate experience in a related field
  • Valid New York State driver's license
  • Excellent communication and interpersonal skills
  • Ability to analyze problems and develop effective solutions
