Constructive Partnerships Unlimited (CP Unlimited) is a leading provider of community-based services for individuals with intellectual and developmental disabilities. We are seeking a dedicated and compassionate Habilitation Assistant to join our team at our Queens Long Island City Day Program.

Job Summary:

The Habilitation Assistant will play a vital role in supporting individuals with disabilities to achieve their goals and live independently. This includes monitoring activities, ensuring safety and well-being, and providing personal care and support as needed.

Key Responsibilities:
  • Monitor and support individuals with disabilities in daily activities, ensuring their safety and well-being.
  • Implement community-based training opportunities, including use of public transportation, travel training, and safety skills.
  • Ensure daily documentation is maintained and use various software for data collection.
  • Provide personal care and support, including feeding, hygiene, and lifting/transferring as needed.
  • Notify management and/or nurse of any medical concerns and accompany individuals to hospital or healthcare provider as needed.
Requirements:
  • High School Diploma or equivalent from an accredited school.
  • 1 year experience working with individuals with intellectual and/or developmental disabilities.
  • Excellent communication and interpersonal skills.
  • Ability to work outdoors for community-based activities as needed.
  • Ability to perform data entry on a personal computer (PC) and MS Word.
  • Criminal background check.
  • Valid NYS Driver's License preferred.
  • Successful completion of a Medication Administration Program.
