Job Title: Habilitation Specialist

At Adapt Community Network, we are seeking a skilled Habilitation Specialist to join our team. As a Habilitation Specialist, you will play a vital role in designing and implementing individualized programs for individuals with developmental disabilities.

Job Summary

The Habilitation Specialist will be responsible for developing and implementing programs that promote community integration and independence for individuals with developmental disabilities. This will involve working closely with individuals, families, and community resources to create personalized plans that meet their unique needs and goals.

Key Responsibilities
  • Develop and implement individualized programs that promote community integration and independence
  • Work closely with individuals, families, and community resources to create personalized plans
  • Supervise and train support staff to ensure high-quality care and support
  • Collaborate with other professionals to ensure comprehensive support and services
  • Monitor and evaluate program effectiveness and make adjustments as needed
Requirements
  • Bachelor's degree in a human services or related field
  • One year of post-graduate experience in a related field
  • Valid New York State driver's license
  • Ability to read, write, speak, and understand English
  • Good oral and written communication skills
  • Good interpersonal skills necessary to interact effectively with coworkers, employees, residents, and their families
  • Good organizational and teaching skills
  • Proficiency in the use of a personal computer and appropriate software
  • Ability to operate wheelchairs and adaptive equipment utilized by individuals served
  • Maintain confidentiality of the records of the individuals served
  • Ability to analyze problems and determine corrective measures
