About the Role

We are seeking a Community Care Liaison to join our team at Trinity Health. As a Community Care Liaison, you will be responsible for promoting the PACE Program to referrals, family/primary contacts, and community agencies the potential referral may be associated with. You will also serve as the contact for newly qualified referrals, providing an overview of the PACE model and obtaining initial information relating to clinical and financial eligibility. This position requires localized travel to referral sources such as hospitals and rehabilitation centers as well as home visits to potential clients. Occasional evening or weekend hours may be required. Work schedule is typically Monday through Friday, 8 am until 5 pm.

The estimated salary for this position is $62,000 - $72,000 per year, depending on experience.

Key Responsibilities

• Serve as contact for prospective enrollees and/or their family/primary parties to provide information about PACE
• Obtain demographic, medical, insurance, financial, and referral information to determine eligibility and interest
• Refer individuals to other appropriate resources when necessary
• Establish and maintain positive relationships with prospective enrollees, which may include visits to homes and/or other settings
• Act as liaison between prospects and staff as necessary, for communication, scheduling, care plan development, and enrollment

Requirements

• Associate's Degree with a minimum of two (2) years of sales or marketing experience preferred
• Valid driver's license
• Ability to work with minimal supervision and exercise independent judgment
• Excellent written and verbal communication and presentation skills
• Comprehensive proficiency with Microsoft product suite (MS Word, Excel, PowerPoint, etc.)
• Ability to type with speed and accuracy
