Nurse Case Manager

Incredible Health is seeking a skilled Nurse Case Manager to join our team in the Winchester, MA area. As a key member of our healthcare team, you will be responsible for conducting comprehensive assessments of patients, developing and implementing individualized care plans, and collaborating closely with healthcare providers and patients to ensure seamless communication and care coordination.
Responsibilities:- Conduct thorough patient assessments, including medical history, medication, treatment plans, and psychosocial needs
- Develop and implement individualized care plans in collaboration with healthcare providers and patients
- Monitor and evaluate patient progress, adjusting care plans as needed and communicating changes to the healthcare team
- Serve as the primary point of contact between patients and healthcare providers, ensuring effective communication and care coordination
- Maintain accurate and up-to-date records and case notes, adhering to confidentiality and compliance standards
- Bachelor of Science in Nursing (BSN) or higher educational attainment from an accredited program
- Active and unencumbered Registered Nurse license
- At least 1 year of experience in case management, preferably in a healthcare or hospital setting
- Exceptional interpersonal and communication skills, both written and verbal, to effectively collaborate with medical professionals, patients, and family members
- Proficiency in electronic health records (EHR) software
- Comprehensive healthcare coverage, including medical, dental, and vision
