Job Summary

Responsibilities
  • Provide direct patient care, including administering medications, performing treatments, and monitoring patient responses.
  • Collect and document patient data, including vital signs, medical histories, and treatment plans.
  • Collaborate with registered nurses and other healthcare professionals to develop and implement patient care plans.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, active licensure as a Licensed Practical Nurse through the Ohio Board of Nursing.
  • Maintain current, active BLS certification through the American Heart Association.
  • IV Certification preferred.
  • Two years recent nursing experience in acute care, long-term care or home health preferred.
  • Valid driver's license.
  • Reliable transportation with proof of insurance.
Working Conditions

This position requires the ability to work in a fast-paced environment, with frequent interruptions and changes in priority. The LPN must be able to lift, bend, and stand for extended periods, as well as push and pull equipment as needed.
