Homecare Nurse

We are seeking a skilled Registered Nurse to join our team in Plymouth, providing high-quality care to patients in their homes. As a Registered Nurse in Home and Community Based Services, you will be responsible for delivering and coordinating patient care, working closely with other healthcare professionals to achieve the best possible outcomes.
Key Responsibilities- Provide high-quality clinical services to patients in their homes, adhering to infection control standards and the plan of care/service plan.
- Conduct thorough clinical nursing assessments to ensure patient eligibility and medical necessity for services.
- Develop and revise individualized plans of care and/or service plans according to federal and/or state program requirements.
- Ensure plan of care frequency and duration meets the needs of the patient and initiates plan of care revisions as needed with physician approval.
- A minimum of one year experience as an RN is preferred.
- Current RN licensure in the state of practice is required.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation are necessary.
- Current CPR certification is required.
