Homecare Nurse Specialist

We are seeking a skilled Registered Nurse to join our Homecare team in Plymouth. As a key member of our team, you will have the opportunity to provide high-quality clinical services to patients in their homes.
Key Responsibilities- Provide comprehensive nursing assessments and develop individualized plans of care in accordance with federal and state program requirements.
- Ensure patient eligibility and medical necessity for services as defined by payer source and agency policy.
- Collaborate with other healthcare professionals to achieve and maintain continuity of patient care.
- Ensure plan of care frequency and duration meets the needs of the patient and initiates plan of care revisions as needed with physician approval.
- A minimum of one year experience as an RN preferred.
- Current RN licensure in state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification required.
