Responsibilities:

* Evaluate patients based on departmental standards
* Conduct and modify evaluations based on patient symptoms, activity level, and medical status
* Treat patients with appropriate therapeutic exercises and modalities
* Educate patients and families on home exercise programs and functional activities
* Document patient care and charges accurately
* Participate in interdisciplinary team meetings and communicate with referring physicians

Requirements:

* Graduate of an accredited Physical Therapy program
* Current Connecticut license in Physical Therapy
* Strong English communication skills
* Personal computer skills and proficiency in Windows-based programs
* Reliable transportation and ability to travel between work sites
