Job Summary

We are seeking a skilled and experienced LPN to join our team of healthcare professionals. As a LPN with Frontier Home Health and Hospice, you will have the opportunity to make a meaningful difference in the lives of our patients and their families. You will visit patients in their homes, providing nursing care under the direction of Registered Nurses (RNs) and in compliance with physicians' orders.

Responsibilities

  • Provide high-quality patient care in a home-based setting, including administering medications, wound care, and vital sign monitoring.
  • Collaborate with RNs and other healthcare professionals to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current with best practices and industry developments.

Requirements

  • Current licensure as a Licensed Practical Nurse in the state of employment.
  • Valid state driver's license and reliable transportation.
  • Ability to drive in all types of weather conditions.
  • Dependable and punctual, with excellent time management skills.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
