Job Summary:

The Licensed Practical Nurse will work under the supervision of a Registered Nurse to provide nursing care to clients in their homes. This includes assessing clients' physical and emotional needs, developing and implementing individualized care plans, and providing education and support to clients and their families.

Responsibilities:
  • Provide direct and indirect nursing care to clients in their homes
  • Assess clients' physical and emotional needs and develop individualized care plans
  • Implement and evaluate care plans, making adjustments as needed
  • Provide education and support to clients and their families
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document client progress and care provided
Requirements:
  • ASN or diploma in nursing
  • Certification/Licensure in the State of Ohio as a Licensed Practical Nurse or licensure from a state participating in the multistate privilege to practice compact with Ohio
  • Minimum of 3 years acute care or home care nursing experience
  • BLS and ACLS Certified
Preferred Qualifications:
  • Strong clinical assessment skills and ability to prioritize and reprioritize
  • Exceptional critical thinking skills
  • Strong communication and interpersonal skills
  • Cultural openness and ability to work with diverse populations
Physical Requirements:
  • Lift and carry equipment/supplies weighing up to 50 pounds long distances
  • Push or pull objects weighing up to 50 pounds
  • Walk long distances and up/down several floors of stairs
  • Twist, bend, stoop/squat frequently throughout the day
  • Reach and grasp with arms and hands, including reaching overhead/above shoulder level, below the waist, and in front of body
