Job Title: Physical Therapist - Home Health

LHC Group is seeking a skilled Physical Therapist to join our team in Gig Harbor, Washington.

Job Summary

We are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Physical Therapist with LHC Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Evaluate patients' functional status and physical therapy needs, and assist physicians in developing therapy plans of care.
  • Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other healthcare team members.
  • Observe and report patients' responses to treatment and any changes in their condition to supervising nurses and/or physicians.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform participating healthcare team members, patients, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
  • Visit patients according to the Plan of Treatment, complete progress notes for each visit, and submit progress notes per LHC policy.
  • Complete all patient evaluations and develop the PT plan of care within state-specific guidelines.
  • Report outcomes of evaluation, goals, and anticipated projected frequency of care.
Requirements
  • Current Physical Therapy licensure in the state of practice.
  • Current CPR certification required.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
