Join Our Team as a Physical Therapist in Home Health

Banner Health is seeking a skilled Physical Therapist to join our Home Health team in Peoria, Arizona. As a Physical Therapist in Home Health, you will have the opportunity to work with patients in their own homes, providing personalized care and rehabilitation services.

About the Role

This position is responsible for evaluating patients' functional abilities and limitations, determining if intervention is needed, and establishing a plan of care and treatment goals. You will work collaboratively with patients, families, and healthcare teams to provide skilled treatment and promote optimal patient outcomes.

Key Responsibilities
  • Evaluate patients' functional abilities and limitations, and determine if intervention is needed
  • Establish a plan of care and treatment goals, and implement individualized treatment plans
  • Provide skilled treatment in a safe manner, and assess ongoing effectiveness of treatment
  • Document patient information according to professional practice guidelines, and communicate with healthcare teams to ensure seamless care
  • Supervise and delegate tasks to physical therapy assistants, and participate in departmental process improvement initiatives
Requirements

To be successful in this role, you must have a graduate degree from an accredited Physical Therapy program, and a current Physical Therapy license in the State of Arizona. You should also possess excellent communication and interpersonal skills, and be able to work effectively in a fast-paced environment.
