Key Responsibilities:
  • Conduct thorough assessments of patients' daily living and work-related skills, developing tailored therapeutic retraining programs to achieve measurable objectives.
  • Collaborate with physicians and other healthcare professionals to develop and revise treatment plans, ensuring seamless care coordination.
  • Provide evidence-based therapeutic interventions, instructing patients on techniques to improve strength, coordination, range-of-motion, and function, or teach compensation measures.
  • Regularly review and expand retraining programs to align with patients' behavioral goals and the rehabilitation program's objectives.
  • Consult with vocational team members to develop vocationally oriented plans, ensuring patients receive comprehensive care.
  • Communicate patient status and progress to physicians, rehabilitation staff, Clinical Managers, patients, and family members, fostering a collaborative care environment.
  • Participate in care coordination and discharge planning activities, serving as a resource for other healthcare professionals to meet patients' needs.
  • Design community reintegration activities to support patients' physical reconditioning and psychological adjustment, coordinating with the interdisciplinary team.
  • Recommend and design special adaptive equipment to enhance patients' living or working environments, or improve their participation in the rehabilitation program.
  • Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills, participating in team and family educational meetings.
  • Supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
  • Accurately document patients' care observations, interventions, and evaluations, ensuring timely submission of evaluation, treatment plans, and discharge summaries to supervisors and care management staff.
Requirements:
  • Degree from an accredited Occupational Therapy Program
  • A minimum of six months of occupational therapy experience preferred
  • Home Health experience a plus
  • Current and unrestricted OT licensure
  • Current CPR certification
  • Good organizational and communication skills
  • Valid driver's license, auto insurance, and reliable transportation
