Mental Health Disability Support Specialist

About the RoleThis is an exciting opportunity to make a difference in the lives of our clients. As a Mental Health Disability Support Specialist, you will be responsible for providing individualized support to our clients to assess and remediate barriers that inhibit their successful integration into the community.
You will work closely with our clients and their families to determine training goals and ongoing success. Your role will involve providing 1:1 assessment, training, and follow-up in the client's home and community, as well as travel destination training.
Essential Responsibilities- Provide 1:1 assessment, training, and follow-up in the consumer's home and community as identified in their ISP (Individual Service Plan).
- Provide travel destination training for clients.
- Assure compliance with safety and confidentiality standards.
- Act as a program monitor for assigned clients, completing required forms and accountability documentation, incident reports, case recordings, termination, client attendance, billing, floor files, and other required documentation.
- Perform task analysis and time studies for wage evaluations as required by DOL.
- Assess and document client progress, performs quality control.
- Maintain all assigned client case records, participating in case reviews and goal planning.
- Schedule transportation, prepare required documentation, reporting.
- Represent HOPE in the community, maintaining a professional relationship with consumers, parents, care providers, co-workers, and others.
- Act as advocate on behalf of clients served, providing intervention and consultation as needed.
- Act as a mandated abuse reporter.
To be successful in this role, you will need:
- A high school diploma, G.E.D., or equivalent, plus one year of related experience working with people with disabilities in a rehabilitation environment.
- The ability to relate positively and constructively with clients, their families, the community, and staff members.
- The ability to observe, evaluate, document, and communicate verbally and in writing client performance and social behavior.
- Knowledge and understanding of physical and mental disabilities, alternative communication systems, and assistive technology.
- Critical thinking skills in decision-making situations, good organizational and record-keeping skills.
- Maintenance of valid CPR and First Aid certification.
- A flexible, positive, and cooperative approach to scheduling and task prioritization in a consumer-driven environment.
This is a dynamic and fast-paced work environment that requires extensive travel in the local community, standing, and walking. You will also be exposed to dust, noise, pollens, and various weather conditions, as well as writing and computer work, and the use of various alternative transportation modes.
