Key Responsibilities- Provide 1:1 patient care to pediatric patients in their homes, promoting a safe and supportive environment.
- Develop and implement individualized care plans, working collaboratively with patients, families, and healthcare teams.
- Administer medications, perform medical procedures, and provide wound care as needed.
- Conduct thorough assessments, documenting patient progress and identifying areas for improvement.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- RN/LPN with a valid license to practice as issued by the State Board of Nursing.
- Current CPR certification.
- Ability to work independently and as part of a team, prioritizing patient needs and safety.
