About the Role

VNA Health Group is seeking a highly skilled Physical Therapist to join our team in a home health setting.

Responsibilities
  • Provide physical therapy evaluation and treatment to patients in their homes or alternate community settings.
  • Assess and evaluate client needs and develop and implement appropriate plans of action in collaboration with the physician.
  • Document all changes in client condition and update care plans and outcomes accordingly.
  • Identify counseling needs and plan appropriate teaching strategies.
  • Monitor quality and appropriateness of services provided.
  • Adhere to agency infection control and safety policies.
  • Adhere to the organization's policy in regards to absenteeism and appearance.
Requirements
  • Physical Therapy Licensure in NJ required.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills.
  • Strong communication and interpersonal skills.
  • Valid NJ drivers license.
