Job SummaryWe are seeking a highly skilled Adult Case Manager to join our Community Support Services team. As a key member of our team, you will be responsible for providing case management services and curriculum-based group sessions to assist seriously and persistently mentally ill adults. Your primary goal will be to locate, coordinate, and monitor necessary care and services to support our clients in achieving their treatment goals.
Key Responsibilities- Provide case management services to adults diagnosed with serious mental illness, including assessment, planning, and coordination of care.
- Develop and implement individualized treatment plans in collaboration with clients, family members, and other healthcare professionals.
- Conduct regular home visits and community outreach to ensure clients receive necessary support and services.
- Collaborate with internal and external providers to coordinate care and ensure seamless transitions.
- Document interventions and progress in electronic medical records, adhering to department standards and regulations.
- Provide education and support in activities of daily living, medication management, and other essential skills.
- Advocate for clients' needs and rights, ensuring they receive fair and equitable treatment.
- Participate in staff meetings, training sessions, and other professional development activities to enhance skills and knowledge.
- Bachelor's Degree in Social Work, Psychology, or related human services discipline.
- Previous experience working in mental health or a related field, preferably with seriously and persistently mentally ill adults.
- Successful completion of OBHP educational requirements within 3 weeks of hire date.
