Clinical Pharmacist

Confluence Health is seeking a highly skilled Clinical Pharmacist to join our team in providing exceptional patient care in an ambulatory setting. As a Clinical Pharmacist, you will play a vital role in the integrated care team, serving as a medication expert and resource to team members.
Key Responsibilities- Medication Management: Monitor medication therapy within the scope of practice as outlined in State law, ensuring optimal patient outcomes.
- Collaboration and Communication: Work closely with the Physician Director or Manager to interpret medical records and test results, adjusting medication therapy in accordance with Confluence Health-approved guidelines and evidence-based practice.
- Patient Assessment and Optimization: Perform ongoing assessment of patient risk factors, indications for therapy, and work towards optimization of therapeutic regimens.
- Documentation and Compliance: Maintain accurate and up-to-date documentation in the electronic medical record, adhering to department standards and regulatory requirements.
- Interprofessional Collaboration: Communicate effectively with patients, family members, and other healthcare providers to promote coordinated care across the continuum.
- Professional Development: Enhance professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
- Departmental Contributions: Participate in department-driven practices, including provider huddles, journal clubs, and on-call scheduling, while adhering to established departmental policies and objectives.
- Certifications and Credentials: Maintain active Basic Life Support (CPR) certification and demonstrate competency in essential job functions.
- Education: Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy degree from an accredited school of pharmacy.
- Licensure: Licensed to practice in the state of Washington prior to start date.
- Technical Skills: Strong computer skills, ability to work in multiple systems simultaneously.
- Experience: Three years of experience as a clinical pharmacist in the ambulatory setting or successful completion of a pharmacy practice residency with ambulatory care experience.
- Certifications: One of the following certifications is preferred and will be required within 24 months of exam eligibility: BCPS, BCACP, CACP, or equivalent certification.
