Confluence Health is seeking a highly skilled and experienced Pharmacist to join our team as a Clinical Services Pharmacist. This role will be responsible for providing clinical and operational pharmacy services to patients in the ambulatory setting.
Key Responsibilities- Therapeutic Assessment and Interventions: Conduct thorough therapeutic assessments of patients to identify medication-related issues and develop plans to improve patient outcomes.
- Medication Management: Manage medication therapy for patients, including ordering labs and entering progress notes as necessary.
- Collaboration with Healthcare Team: Work closely with healthcare providers and other members of the interdisciplinary care team to ensure coordinated care and optimal patient outcomes.
- Quality Improvement: Participate in quality improvement initiatives to enhance medication safety and patient care.
- Education and Training: Provide education and training to patients, family members, and healthcare staff on medication-related topics.
- Leadership and Supervision: Provide direct supervision and oversight to clinical staff members and participate in departmental performance initiatives.
- Education: Bachelor of Science in Pharmacy (RPh) or Pharm.D.
- Licensure: Licensed to practice in the state of Washington prior to start date.
- Experience: Three years of experience as a clinical pharmacist in the ambulatory setting or successful completion of a pharmacy practice residency with ambulatory care experience.
