Job Description

We are seeking a skilled Nurse Care Coordinator to join our team at Methodist Hospital Northeast. As a key member of our cardiovascular services team, you will be responsible for coordinating patient care and ensuring that patients receive high-quality, personalized attention.

The ideal candidate will have 1-2 years of experience in cardiovascular telemetry or PCU/medical surgical acute care hospital nursing. A bachelor's degree in nursing is preferred, along with basic cardiac life support certification within 30 days of employment start date. ACLS certification is also required.

As a nurse care coordinator, you will partner with a multidisciplinary team to customize and deliver high-quality patient-centered care. You will assist with admission and discharge processes, provide coaching and feedback to improve performance, and help staff scheduling. Your role will involve assessing patient condition during admission and each shift, identifying changes in patient status, and reporting them to the medical team.
