Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at KPH Healthcare Services. As a Clinical Pharmacist, you will play a critical role in providing quality products and services to our infusion therapy patients.

Key Responsibilities
  • Compliance and Regulatory Expertise
    • Ensure compliance with federal, state, and local laws and regulations, as well as accrediting body standards.
  • Medication Order Review and Evaluation
    • Interpret and evaluate physician's medication orders to ensure therapeutic appropriateness, cost-effectiveness, accuracy, and completeness.
  • Record Keeping and Documentation
    • Accurately maintain all legally required records.
  • Supervision and Training
    • Directly supervise pharmacy support personnel and/or interns in the preparation, packaging, and labeling of pharmaceuticals.
    • Assist in training and orientation of new employees.
  • Clinical Information and Communication
    • Provide technical and clinical information to patients, nurses, and physicians regarding the contents, utilization, therapeutic actions, possible adverse reactions, or interactions of drugs.
  • Smart Meds Pharmacy Documentation
    • Initiate, review, and update all Smart Meds Pharmacy required documentation per Policy and Procedure.
  • Inventory Management and Procurement
    • Follow established procedures for DME and supply procurement and maintain appropriate inventory levels and preferred product selection of drugs, solutions, and supplies.
  • IV Admixture Preparation
    • Accurately compound IV admixtures, utilizing appropriate technique, performing checks and double checks, per Policy and Procedure.
  • Quality Control and Stability
    • Adhere to stability and compatibility guidelines and quality control procedures.
  • Security and Compliance
    • Maintain procedures for appropriate security of all legend items, including controlled substances.
  • Computer Order-Entry and Dispensing
    • Perform accurate computer order-entry, labeling, and dispensing.
  • Clinical Activities and Collaboration
    • Participate with nurses and physicians in all clinical activities, including start of care prospective review of medical and drug histories, pharmacokinetic dosing, dosage monitoring for compromised patients, reviewing laboratory test results, attending patient care conferences, developing and updating patient-specific plans of care (POC), patient discharge consultation.
  • Referral Acceptance/Denial and Product Selection
    • Participate in the referral acceptance/denial process and product, device, and supply selection.
  • On-Call Rotations and Performance Improvement
    • Participate in on-call rotations.
    • Participate in Performance Improvement activities, educational programs, and departmental/branch meetings.
  • Primary Case Management Model (PCMM)
    • Drive the Primary Case Management Model (PCMM) in daily interactions through the delegation of non-cognitive activities, such as lab packing to support personnel, ordering only clinically necessary and cost-effective labs, and facilitation of positive patient outcomes with patients, nursing agencies, and other healthcare providers.
  • Additional Responsibilities
    • Perform other duties as assigned.
    • Serve as Process Owner as assigned.
    • Serve as compliance officer.
    • Responsible for completing all mandatory and regulatory training programs.
