Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Option Care Health. As a Clinical Pharmacist, you will play a critical role in providing safe and effective pharmacy services to our patients in the comfort of their own homes.

Key Responsibilities
  • Assess patients' suitability for home care services and develop personalized pharmacy care plans
  • Design, recommend, and evaluate patient-specific pharmacy care plans for the pharmacotherapy of home care patients
  • Provide medication-use education to Option Care staff, other professionals, home care patients, and/or their caregivers
  • Ensure continuity of pharmaceutical care to and from the home and other patient-care settings
  • Prepare and dispense medications using appropriate techniques and following Option Care policies and procedures
Requirements
  • Graduate of an accredited School of Pharmacy
  • Current, active Pharmacy license as granted by the State Board of Pharmacy
  • At least one year of experience in sterile IV admixture procedures, knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM), or completion of an ASHP accredited Pharmacy Residency Program
