About the Role

We are seeking a compassionate and skilled Hospice Care Assistant to join our team at Abode Hospice and Home Health. As a Hospice Care Assistant, you will play a vital role in providing high-quality care to patients in their homes, supporting them with their daily needs and promoting their overall well-being.

Key Responsibilities
  • Provide personal care services to patients, including bathing, dressing, and grooming
  • Assist patients with daily activities, such as meal preparation and ambulation
  • Monitor patients' conditions and report any changes to the Case Manager
  • Collaborate with the hospice team to develop and implement individualized care plans
  • Provide emotional support and companionship to patients and their families
Requirements
  • Completed a minimum of 75 hours of classroom and supervised practical training
  • Minimum of 6 months experience in providing personal care
  • Ability to read and follow written instructions and document care given
  • Ability to work with little direct supervision
  • Current CPR certification
