Hospice Care Specialist

As a Hospice Care Specialist, you will play a vital role in supporting patients and families by providing comprehensive hospice care services. Your expertise will be invaluable in evaluating patient medical conditions, developing plans of care, and prescribing medication and treatment. Responsibilities
Evaluating patient medical conditions and developing plans of care
Prescribing medication and treatment as needed
Collaborating with interdisciplinary teams to meet patient and family needs
Ensuring compliance with state and federal regulations
Qualifications
Graduate from an accredited School of Nursing and accredited Nurse Practitioner program
Current DEA license, if applicable to state regulations
Current nursing license in state of practice
Master's degree with minimum of one year NP experience
One year hospice or palliative care experience preferred
