Position Summary:
As an RN Care Coordinator, you will play a pivotal role in our Integrated Care Model by communicating essential information regarding member needs and advocating for optimal care solutions. You will ensure that members receive the appropriate services tailored to their requirements. Our team comprises RNs, LVNs, and CACs committed to supporting members in achieving their health objectives.

Key Responsibilities:
  • Evaluate, plan, and execute personalized care strategies aimed at providing the most suitable and least restrictive level of care.
  • Identify and initiate referrals for member transitions to community resources and assist with social service programs, including financial, psychosocial, and state-supported services.
  • Oversee the care plan throughout the care continuum, serving as a single point of contact.
  • Facilitate communication among all stakeholders to ensure timely delivery of quality coordinated care and services.
  • Advocate for members and their families to ensure their needs and preferences are fully represented and supported by the health care team.

Travel Requirements:
This position requires local travel approximately 50% of the time based on business needs.

Qualifications:
Required:
  • Current, unrestricted RN license in Texas.
  • Minimum of 2 years of experience in a community health setting or a related health care role.
  • Proficiency in Microsoft Office Suite, including Word, Excel, and Outlook.
  • Reliable transportation and the ability to travel within the assigned region.
  • Access to high-speed internet at home.

Preferred:
  • At least 1 year of experience with long-term care services and support, Medicaid, or Medicare.
  • Ability to create, edit, save, and send documents, spreadsheets, and emails.
  • Familiarity with integrated care settings and knowledge of federal and state regulations.
  • Reside within a reasonable commuting distance of assigned duties.
