Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Levindale Hebrew Geriatric Center and Hospital. As a member of our post-acute services team, you will play a vital role in providing high-quality care to our residents.

Responsibilities
  • Develop and implement individualized nursing care plans to meet the unique needs of our residents
  • Provide direct patient care, including administering medications, performing vital signs, and assisting with daily living activities
  • Collaborate with the interdisciplinary team to ensure seamless care transitions and optimal patient outcomes
  • Participate in quality improvement initiatives to enhance the overall quality of care
Requirements
  • Graduate of an accredited school of nursing with a current LPN license
  • Basic computer skills and experience with electronic medical records
  • Ability to work effectively in a fast-paced environment and prioritize tasks efficiently
  • Strong communication and interpersonal skills, with a focus on patient-centered care
