Nurse Case Manager

We are seeking a skilled Registered Nurse to join our Case Management team at Conemaugh Memorial Medical Center. As a key member of our interdisciplinary team, you will play a vital role in coordinating patient care and ensuring seamless transitions between hospital and community settings.
Key Responsibilities- Develop and implement individualized plans of care for patients, taking into account their unique needs and goals.
- Collaborate with healthcare providers, social workers, and other stakeholders to ensure comprehensive care coordination.
- Conduct thorough assessments of patients' medical and social needs, identifying opportunities for improvement and developing strategies to address them.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal outcomes.
- Stay up-to-date with the latest clinical guidelines, research, and best practices in case management, applying this knowledge to improve patient care and outcomes.
- Current state RN license and an associate's or bachelor's degree from an accredited nursing school.
- At least 3 years of experience in acute care as a Professional Nurse or RN experience with significant acute care hospital Case Management experience.
