Nurse Case Manager

We are seeking a skilled Registered Nurse to join our Case Management team at Conemaugh Memorial Medical Center. As a key member of our interdisciplinary team, you will play a vital role in coordinating patient care and ensuring seamless transitions between hospital and community settings.
About the RoleThis is a full-time position that requires a strong background in acute care nursing, with a focus on case management principles and practices. You will work closely with patients, families, and healthcare providers to develop and implement individualized care plans that promote optimal health outcomes.
Responsibilities- Conduct comprehensive assessments of patients' medical and psychosocial needs
- Develop and implement individualized care plans that address patients' unique needs and goals
- Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between hospital and community settings
- Provide education and support to patients and families to promote self-management and empowerment
- Participate in quality improvement initiatives to enhance patient care and outcomes

RequirementsTo be considered for this role, you must have a current state RN license and a minimum of 3 years of experience in acute care nursing. A bachelor's degree in nursing or a related field is preferred. You should also possess strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
