Nurse Case Manager

We are seeking a skilled Registered Nurse to join our team at Conemaugh Memorial Medical Center. As a key member of our healthcare team, you will play a vital role in ensuring the highest level of care for our patients.
About the RoleThis is a full-time opportunity for a Registered Nurse to work in a dynamic and supportive environment. As a Nurse Case Manager, you will be responsible for performing pre-admission, admission, and continued stay reviews to ensure accurate status documentation and registration. You will also substantiate medical necessity and intensity of service based on industry-standard criteria and ensure appropriate resource utilization throughout the hospital course.
Responsibilities- Perform pre-admission, admission, and continued stay reviews to ensure accurate status documentation and registration
- Substantiate medical necessity and intensity of service based on industry-standard criteria
- Ensure appropriate resource utilization throughout the hospital course
- Proactively communicate with physicians, patients, and families, third-party payers, and allied health professionals regarding medical necessity issues
- Refer cases to Behavioral Medicine Psychiatrist for medical management intervention
- Diploma, Associate, or Bachelor's Degree in Nursing
- Current PA Registered Nurse License to practice nursing in the Commonwealth of PA
- One year experience as a Psychiatric RN or experience with Case Management
- Clinical Nursing experience in Behavioral Medicine preferred
- Behavioral Medicine Case Management experience preferred
