About the Job

This position involves working closely with our members, caregivers, and healthcare professionals to assess, plan, and coordinate care across the continuum. You will be responsible for developing and implementing individualized care plans that address the unique needs and goals of each member.

Key Responsibilities
  • Conduct comprehensive assessments of members to identify their needs and goals
  • Develop and implement personalized care plans that address medical, behavioral, and social needs
  • Collaborate with multidisciplinary teams to ensure seamless care coordination
  • Provide education and support to members and caregivers to promote health and well-being
  • Evaluate and authorize waiver services as needed
Requirements

To be successful in this role, you will need:

  • Bachelor's or master's degree in a related field (e.g., social work, psychology, gerontology)
  • At least 1 year of experience working with individuals with disabilities or chronic conditions
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaboratively as part of a team
Benefits
