**Job Title:** Healthcare Support Specialist

Lakeland Care is seeking a detail-oriented and organized Healthcare Support Specialist to join our team. As a Healthcare Support Specialist, you will play a critical role in supporting our interdisciplinary team to provide high-quality care to our members.

You will be responsible for maintaining accurate records, coordinating care management services, and implementing risk mitigation strategies to promote members' health, safety, and independence. Your strong analytical skills and experience working in a fast-paced environment will enable you to make a significant contribution to our team.

Key Responsibilities:

  • Maintain accurate and up-to-date records of member interactions and service delivery.
  • Coordinate care management services to ensure timely and effective delivery of services.
  • Implement risk mitigation strategies to promote members' health, safety, and independence.
  • Participate in team meetings and training sessions to stay up-to-date on policies and procedures.

Requirements:

  • Associate's degree in Health Information Technology or related field.
  • Minimum two years of experience working in a healthcare setting.
  • Strong analytical and organizational skills.
  • Ability to work independently and as part of a team.
