Job Summary

The Community Health Worker plays a vital role in helping patients and their families access community resources, providing education and support to promote healthy behaviors and disease prevention.

Key Responsibilities
  • Provide community outreach and connect patients to resources and services to address their social needs
  • Support providers and care coordinators through an integrated approach to care management and community outreach
  • Educate clients on disease prevention and make referrals to assist with coordinating services
  • Assist clients in self-management of chronic illnesses and medication adherence
  • Organize and facilitate support groups to promote patient engagement and well-being
Requirements
  • High School Diploma or GED
  • +1 year of experience in serving as a community health advocate
  • Certification as a Community Health Worker in Texas or formal training
  • + A resident of Lubbock County for at least 1 year
Skills and Abilities
  • Excellent customer service skills and the ability to resolve complex customer service issues
  • Ability to read, write, and speak English in a clear, concise, professional manner
  • Proficient computer skills and knowledge of various software
  • Demonstrated ability to work with diverse and cultural differences amongst people
Working Conditions

This position requires the ability to work in a well-lighted, heated and ventilated building and to do home visits, therefore, dependable transportation is required.
