Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Home Health RN. As a Home Health RN, you will be responsible for providing high-quality patient care in the comfort of their own homes. You will work closely with patients, families, and healthcare teams to develop and implement personalized care plans that meet the unique needs of each individual.

Key Responsibilities
  • Conduct thorough assessments of patients to identify their healthcare needs and develop effective care plans
  • Provide skilled nursing care, including wound care, medication management, and education on disease management and self-care
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and coordination
  • Document patient progress and outcomes in a timely and accurate manner
  • Participate in ongoing education and training to stay current on best practices and industry developments
Requirements
  • Current RN licensure in the state of Texas
  • 2+ years of experience in home health care or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic computer skills and proficiency in electronic medical records
Preferred Qualifications
  • Bachelor's degree in nursing
  • Experience working with electronic medical records and other healthcare software
  • Knowledge of current nursing practices and industry regulations
