Job Summary

We are seeking a skilled and compassionate Home Health Nurse to join our team. As a Home Health Nurse, you will provide direct patient care, administer medications, and educate patients and their families on disease management and self-care techniques.

Responsibilities
  • Provide direct patient care, including administering medications, treatments, and therapies
  • Document all activities, assessments, and nursing actions
  • Educate patients and their families on disease management and self-care techniques
  • Collaborate with other healthcare professionals to coordinate care and achieve optimal patient outcomes
Qualifications
  • Active Nursing License in good standing
  • Six (6) months of recent experience as a Nurse in a clinical care setting
  • Home health experience preferred
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
