Position Summary:

The Speech Language Pathologist (SLP) will deliver coordinated services to clients across all age groups. Responsibilities include planning, implementing, and assessing care strategies to promote or sustain patient health. The SLP will evaluate the functional roles and capabilities of clients facing physical, psychological, and cognitive challenges, providing targeted interventions to address identified deficits.

Key Responsibilities:

  • Conduct clinical evaluations to determine patient needs and suitability for services.
  • Document, prepare, and update the Plan of Care (POC), progress notes, and other necessary clinical records consistently.
  • Assess client requirements and deliver appropriate interventions as prescribed by a physician.
  • Implement infection control measures and universal precautions during client care activities to ensure safety for both clients and staff.
  • Communicate effectively with clients, their families, team members, and other healthcare professionals, actively participating in case discussions.
  • Monitor assigned cases to ensure adherence to third-party payer regulations.
  • Commit to professional development by engaging in training programs and enhancing skills.

Qualifications:

Experience:

  • Minimum of two (2) years of experience as an SLP in a clinical setting.
  • Home health experience is preferred.

Skills:

  • Proficient SLP skills aligned with established standards of practice.
  • Strong interpersonal abilities and effective communication skills.
  • Demonstrated decision-making capabilities.
  • Fluency in reading, writing, and understanding English.

Education:

  • Master's or Doctorate degree in Speech-Language Pathology from an accredited institution.
  • Licensure/Certification:
  • Valid driver's license in good standing, with responsibility for timely renewal.
  • Reliable transportation and auto liability insurance.
  • Current state professional license to practice as an SLP in applicable states.
  • Current CPR certification.

Ongoing employment as an SLP necessitates maintaining a valid license and CPR certification, with the responsibility for timely renewals.

Physical Requirements:

  • Ability to stand and walk for extended periods.
  • Capability to lift and transfer up to 50 lbs.
  • Ability to meet deadlines and patient needs, make prompt decisions, and allocate resources effectively under pressure.
  • Address individualized psychosocial needs of patients and families.
  • Requires manual dexterity and hand-eye coordination.

Work Environment:

  • Services are provided in clients' homes under varying conditions.
  • Potential exposure to blood-borne pathogens, bodily fluids, and infectious diseases.
  • Some exposure to adverse weather conditions; on-call emergency duties may be required.
  • Ability to travel locally and maintain a flexible schedule.
