We are seeking a highly skilled Registered Dietitian to join our team at Carlisle Nursing & Rehab. As a key member of our interdisciplinary team, you will be responsible for providing expert nutrition care to our residents, ensuring their overall health and well-being.

Key Responsibilities:
  • Develop and implement individualized nutrition plans of care for residents, taking into account their medical history, dietary needs, and preferences.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and effective communication.
  • Conduct regular assessments and evaluations to monitor the effectiveness of nutrition interventions and make adjustments as needed.
  • Provide education and support to residents, families, and caregivers on nutrition-related topics, including meal planning, food safety, and healthy eating habits.
  • Stay up-to-date with the latest research and best practices in nutrition care, applying this knowledge to improve patient outcomes and enhance the quality of care.
Requirements:
  • Registered Dietitian (RD) or licensed dietitian, as required by the state of employment.
  • Previous experience in a healthcare setting or as a consulting dietitian, with a preference for experience working with geriatric populations.
