Job Summary

We are seeking a highly skilled Registered Dietitian to join our team at Carlisle Nursing & Rehab in Carlisle, PA. As a key member of our interdisciplinary team, you will be responsible for providing comprehensive nutrition care to our residents, including developing and implementing nutrition plans, conducting assessments, and educating patients and families on healthy eating habits.

Key Responsibilities:
  • Collaborate with the interdisciplinary team to develop and implement individualized nutrition plans for residents
  • Conduct regular assessments to evaluate the nutritional needs of residents and make recommendations for improvement
  • Provide education and counseling to residents and their families on healthy eating habits and nutrition-related topics
  • Monitor and evaluate the effectiveness of nutrition interventions and make adjustments as needed
  • Ensure accurate and timely documentation of nutrition assessments, plans, and interventions
Requirements:
  • Registered Dietitian (RD) or licensed dietitian (LD) as required by the state of employment
  • Previous experience in a healthcare setting or as a consulting dietitian preferred
  • Experience working with geriatric populations a plus
