Position Title: Case Manager
Compensation Range: $52,000 - $56,000
Position Overview
The Case Manager plays a crucial role in conducting comprehensive assessments of students' needs and identifying available resources to assist them. This position involves managing caseloads, making referrals, setting goals, and acting as an advocate/facilitator to address student requirements.
Key Responsibilities include, but are not limited to:
- Managing student cases to alleviate barriers and fulfill basic needs, offering services through Evergreen Goodwill or via referrals.
- Delivering personalized services, workshop content, and referrals to students through in-person meetings or remote service delivery via video conferencing. Regularly conducting workshops on essential needs and resource topics.
- Providing advocacy and following up with service providers regarding referrals. Addressing challenges that arise and offering solution-focused strategies to help referrals access support services, while ensuring current clients maintain eligibility for these services.
- Completing intake assessments, including all necessary forms and documentation for student enrollment.
- Maintaining precise and accurate case notes in the database and securely managing both paper and electronic student files. Collecting, monitoring, and reporting on student outcome data as applicable.
- Building professional relationships with students and Center staff to facilitate successful outcomes.
- Collaborating with students, staff, and partners to implement initiatives centered around student needs.
- Presenting relevant case-related information through reports or meetings.
- Actively participating in staff meetings and contributing to departmental projects.
- Networking with community-based agencies, organizations, and groups to stay informed about local services and resources.
- Demonstrating safe work practices through awareness and observation in support of a safety culture; reporting any potential hazards or incidents.
- Performing additional duties as assigned.

Education and Experience:
- Master's or Bachelor's degree in Social Work or a related field, or an equivalent combination of experience and education. A Master's Degree is preferred; however, lived experience and international experience may be considered in lieu of more traditional U.S. credentials.
- A minimum of two (2) years of experience in social work, human services, or case management is required.
- 2-3 years of experience in social services involving interviewing, counseling, or crisis intervention.
- Familiarity with local community resources and agencies, as well as government benefit and immigration policies and procedures.
- Experience with remote service delivery is a plus.
- Ability to communicate and work effectively with students from diverse economic, cultural, and ethnic backgrounds, as well as those with physical and mental abilities and limited English proficiency.
- Multi-lingual skills are a plus.
- Demonstrated commitment to equity and non-discrimination in practice and execution of duties.
- Ability to read and interpret documents such as law enforcement reports, government or agency regulations, and legal interpretations.
- Ability to maintain confidentiality.
- Skilled in de-escalation techniques.
- Ability to work independently and collaboratively as part of a team. Capable of making sound decisions without on-site supervision.
- Remote work and occasional evening and weekend availability are required.
- Must be able to meet travel requirements of up to 20% of work time locally and statewide.
- Must possess a valid driver's license and proof of insurance.
- CPR/First Aid certification is preferred.
- Highly proficient with web-based applications (Google Workspace), video conferencing tools, case management software, and Microsoft Office.
- Ability to learn new technology and software relevant to the job.
While performing the duties of this position, the employee will be required to sit for extended periods, communicate with colleagues through speaking and listening, and operate computer systems. The ability to safely lift/push/pull up to 25 lbs., grasp, handle, and manipulate items is necessary; performing repetitive fine motor functions with fingers, wrists, and arms, and standing for prolonged periods is also required. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Effective communication with others, including those with limited English skills, is essential. Reliable attendance is expected.
