Job Summary:

As a RN Case Manager at HCA HealthONE Swedish, you will be responsible for coordinating services through an interdisciplinary process, providing a clinical and psychosocial approach to the continuum of care. You will assess patients to determine appropriateness of admission, continued hospitalization, and appropriate level of care. You will conduct discharge planning, beginning at the time of admission, and reassess ongoing throughout the course of hospitalization.

Responsibilities:

  • Coordinate services through an interdisciplinary process, providing a clinical and psychosocial approach to the continuum of care.
  • Assess patients to determine appropriateness of admission, continued hospitalization, and appropriate level of care.
  • Conduct discharge planning, beginning at the time of admission, and reassess ongoing throughout the course of hospitalization.
  • Monitor Quality and Risk Management issues and report to the QRM department as a part of the CQI process.

Requirements:

  • Current licensure in the State of Colorado as a Registered Nurse, or current active multistate nursing licensure.
  • Graduate of an accredited school of nursing.
  • Ability to compile, evaluate, and report statistics to members of the team, and utilize the information to facilitate process improvement activities.
