We are seeking a highly skilled and compassionate Palliative Care Nurse Practitioner to join our team at Gentiva Hospice. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to individuals with serious illnesses.
Key Responsibilities- Provide comprehensive, individualized treatment plans for patients with complex needs, working collaboratively with the Palliative Medical Director and other team members.
- Conduct thorough assessments, including disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately controlled pain or other distressing symptoms, falls, infections or similar events, and the impact of the disease burden on the patient's condition, caregiver and/or family's quality of life.
- Establish and maintain effective working relationships with supportive care team members, as applicable, and involve team members when patient care plans warrant team participation to achieve desired outcomes.
- Communicate clinical findings to the Palliative Medical Director as needed, but no less than bi-weekly, and re-assess the effectiveness of care plans on a regular basis, modifying as needed.
- Prescribe medications, including controlled substances, to the extent delegated and licensed, in accordance with state law, and order and interpret laboratory and other diagnostic tests within the scope of professional practice.
- Master's degree in Nursing from an accredited school of nursing, or an equivalent as allowed under state law.
- Minimum of two years of Nurse Practitioner experience, with a specialty in Family or Adult Medicine preferred.
- Current license as a registered nurse or equivalent as required by state law for Nurse Practitioner licensure.
- Current Nurse Practitioner certification/license, and a collaborative agreement in place with a supervising physician agreeable to supervise work in palliative care program, or must agree to enter a collaborative agreement with Palliative Medical Director, as required by state law.
- Part B Provider certification, Federal DEA license, and State DEA license (if required).
- ACHPN - Certification in Hospice and Palliative Care, preferred.
