About the Role

Gentiva Hospice is seeking a skilled Palliative Care Nurse Practitioner to join our team. As a Palliative Care Nurse Practitioner, you will play a vital role in providing comprehensive, individualized care to patients and their families. You will work collaboratively with the Palliative Care Medical Director and other team members to deliver high-quality care and support.

Key Responsibilities
  • Assess patients' needs and develop individualized care plans
  • Provide diagnosis, treatment, and follow-up care to patients
  • Collaborate with the Palliative Care Medical Director and other team members to ensure seamless care delivery
  • Communicate effectively with patients, families, and healthcare teams
  • Document patient care and outcomes in a timely and accurate manner
Requirements
  • Master's degree in Nursing from an accredited school
  • Current license as a registered nurse or equivalent
  • Current Nurse Practitioner certification/license
  • Collaborative agreement with a supervising physician
  • Part B Provider certification
  • Federal DEA license
  • State DEA license (if required)
