Key Responsibilities
  • Advocacy and Support: Advocate for and with our members to ensure informed decision making, informed consent, and appropriate guardianship.
  • Life Plan Development: Schedule, lead, and collaborate with members and their interdisciplinary team to conduct meetings and assessments, ensuring the development of a comprehensive, person-centered Life Plan that reflects the person's needs and desired life goals.
  • Life Plan Implementation: Implement, update, and monitor Life Plans, and facilitate individualized Life Plan reviews and approval processes.
  • Support Integration: Ensure integration of all needed and preferred supports and services, including medical, behavioral, social, habilitation, dental, psychosocial, and community-based, and facility-based long-term supports and services.
  • Ongoing Support: Maintain ongoing contact with critical people in a member's life, ensure timely submission of all documentation, and assist members with maintaining benefits such as Social Security, Supplemental Security Income, Medicaid, and Medicare coverage, and Food Stamps.
  • Problem Resolution: Assist members to resolve problems in living, such as housing, utilities, the judicial system, and general safety.
  • Reporting: Report abuse or neglect immediately when observed or reported.
Qualifications
  • A Bachelor's degree with 2+ years of relevant experience, OR
  • A License as a Registered Nurse with 2+ years of relevant experience, which can include any employment experience and is not limited to case management/service coordination duties, OR
  • A Master's degree with 1+ year of relevant experience.
  • Strong communication skills, including verbal and written communication skills, along with strong interpersonal and organizational skills.
  • Excellent organizational, interpersonal, and verbal and written communication skills.
  • Bilingual in English and Yiddish preferred.
